Baby Eagan - NICU Day 58 - Sepsis



 
  Once again, Eagan couldn't keep the calm lasting long. He is now showing signs of sepsis. The doctors have put him on a regimen of antibiotics to treat this. His ventilator settings are drastically increased and he just doesn't seem quite himself.
Unrelated, he will also need another surgery next week to place a THIRD broviac central line. Vein access continues to be a major issue with Eagan.
Weight
3 lb 12 oz - He is up 3 ounces from yesterday, more swelling.
Breathing
Eagan is on the conventional ventilator. His settings are up to 45 breaths per minute. His oxygen rate has been around 28%. The pressure that the ventilator puts in his lungs was turned up to 23. Also, he has fluid in his lungs so he is getting Atrovent breathing treatments every six hours. They try to use an end-tidal to monitor his carbon dioxide, but his swelling makes it impossible for the machine to work properly.
Vein and Artery Access
Eagan has an internal jugular line that was placed in surgery. Only one of the two lumens is usable now.

His arterial line stopped working yesterday.

Eagan has an IV in his head.
 
Medications
Two new antibiotics for sepsis were started, Vancomycin and Gentamicin.
 
An antibiotic, Oxicillin is being given every six hours.
Hydrocortisone, for blood pressure and stress response, is down to 1.5 mg.
 
His Morphine drip was turned off and he is now on Morphine every two hours.

Caffeine has been started as a diuretic to try to speed up his kidney function.
He is also on Diflucan as a preventative measure.

Eagan also is back on the Synthroid for low thyroid.  
 
He is also on Zantac to help calm his tummy's acid levels since he isn't eating.

Lasix, a diuretic, was given after the transfusion.
Nutrition
Eagan's only sources of nutrition right now are his TPN fluids and lipis
Output
Lots of pee and a little poop today.

Transfusion #26 of blood today


Upcoming Tests
Another eye exam to check on the severity of his retinopathy of prematurity will be in less than two weeks.

Glad they caught it early. He will fight through this like everything else. Continued prayers for baby Eagan.
