Baby Eagan - NICU Day 62 - Two Months Old with a Bang

  Eagan is two months old. He decided to mark this event with a bang. This morning, his carbon dioxide levels were over 100 on the end-tidal and he dropped his oxygen to 14. The doctors ordered and X-ray. His lungs are completely "whited-out" or full of fluid and his pleural effusions are still there. The decision was made to give him Norcuron to paralyze him. This would enable the ventilator to work properly rather than Eagan fighting it. His feedings were also stopped. Vein access is still a major issue. A PICC nurse tried a few times for a PICC line again today, but the catheter just won't thread. The doctors and surgeons are discussing options now.

Weight
3 lb 8.5oz


Breathing
Eagan is on the conventional ventilator. His settings were up to 60 breaths per minute this morning but he has since weaned to 49. His oxygen rate was at 60%-50% but he is now only on about 30%. The pressure that the ventilator puts in his lungs is up to 24. Also, he has fluid in his lungs so he is getting Atrovent breathing treatments every six hours. He still has the end-tidal to monitor is carbon dioxide levels.








Vein and Artery Access 
Eagan has an internal jugular line that was placed in surgery. Only one of the two lumens is usable now.



No IVs. 

He now has an arterial line in his left foot.

 



Medications


An antibiotic, Oxicillin is being given every six hours. 

 




Hydrocortisone, for blood pressure and stress response, is down to 1.38 mg.
 


He is now on Morphine every four hours.

Caffeine has been started as a diuretic to try to speed up his kidney function.

 


He is also on Diflucan as a preventative measure.

Eagan off of the Synthroid for low thyroid.  

 


He is also on Zantac to help calm his tummy's acid levels since he isn't eating.


 





Nutrition
Feedings were stopped today.







Output

Lots of pee and poop today.

Upcoming Tests

Another eye exam to check on the severity of his retinopathy of prematurity will be this week.



Cranial ultrasound at 36 weeks gestation.
