Baby Eagan - NICU Day 89-92 - Surgery 7 & 8

Day 89 - Intubated again.

Day 89 - On CPAP
Hello poop, meet fan. This is how we have felt the past four days. My extubated, eating baby became a ventilated, NPO baby very quickly.  He is definitely septic with four days of positive cultures. The bacteria has been identified as enterobacter. Honestly, that means very little to me, but the doctors are happy to know what the funk is that is causing the issues. It has also been thought that Eagan has a septic ileus because is body is working so hard to fight the infection, it isn't keeping his intestines working. Because his intestines stopped working, his belly is very distended.
On Day 90 his femoral line was removed, as it was most likely the cause of the infection. On Day 91, the surgeon tried to place another femoral line. This surgery was done in Eagan's room. The vein would not accept the line. Later that day, Eagan was taken to the operating room and a sub-clavian line was placed. This is now on the left side just below his collar bone.
Weight
Day 89 - 3 lb 15 oz
Day 90 - Eagan was too sick to be weighed.
Day 91 - 4 lb 6 oz
Day 92 - 4 lb 10 oz

Breathing
Day 89 - Eagan was taken off the nasal cannula and put on CPAP. He was intubated later that night on a rate of 40 breaths per minute, 9.9 tidal volume and 40% oxygen. It also took two attempts to intubate him because of a mucous plug.
Day 90 - His ventilator was turned up to 50 breaths per minute and a tidal volume of 10.
Day 91 - Ventilator was turned down to 46 breaths per minute.
Day 92 - Settings are down to 43 breaths per minute.

Vein and Artery Access 
Eagan had a femoral line in his upper thigh. It was removed due to his sepsis on Day 90. He now has a sub-clavian line on the left side as of Day 91.

Day 90 - IV in right foot and left hand

Medications

He is back on Morphine every 6 hours as needed.

He was started on Vancomycin and Gentamicin for suspected sepsis.

Vancomycin was discontinued today.

Ampicillin was started in place of the Vancomycin.

Caffeine has been started as a diuretic to try to speed up his kidney function.

He is also on Diflucan as a preventative measure.

He is also on Zantac to help calm his tummy's acid levels.

Lasix, a diuretic, will be given every 24 hours.

He is back on Synthroid for low thyroid.

Eagan was given sodium bicarbonate multiple times for being acidodic.

On Day 90, Eagan had an infusion of gamma globulin.

Nutrition

Feeds have been stopped due to sepsis.  He is getting TPN and lipids.

July 11 - Transfusion #31 of platelets

July 10 - Transfusion #30

  

Upcoming Tests
His eye exam Monday was cancelled because he is so sick. Next Tuesday will be his eye exam.

I was so excited to read about the extubation and although he got reintibated, there is hope that he will take the next extubation just as smoothly! He looks so cute in those clothes. I loved those green socks!!! Such a handsome little man! Im glad u got to spend time with ur other little ones too. Remember to take care of yourself too momma!! I will continue to keep y'all in my prayers....WTE July momma....
( c section scheduled for July 18th...can't wait!)
